Bring
your insurance card.
Bring your co-pay and/or account balance.
Write
down your primary concern. Secondary
concerns will be addressed if time allows.
When scheduling, please ask for a longer
appointment if
you have a number of concerns.
Bring
blood sugar and blood pressure records if appropriate.
Plan
