accept most major non-HMO insurance plans and Medicare.
Please contact us to determine whether we accept your
plan. If we don't currently accept your plan we may
be able to become credentialed with it.
Please
bring your insurance card to each appointment.
Insurance
co-payments are due at each appointment.
We
will gladly file claims with your insurance company.
We
will work on a payment plan with you if you cannot pay
your bill. Late and past due fees will apply to payment
plans.
