Insurance
co-payments are due at each appointment.
If
you do not have health insurance, full payment is due
at each appointment. You will be charged a flat fee of $70 plus 60% of any tests or procedures performed.
We
will work on a payment plan with you if you cannot pay
your balance in full. Late and past due fees will apply to payment
plans.
We
accept cash, MasterCard & Visa (credit or debit
cards), and personal checks.
A
$25 fee will be charged on all returned checks.
Persistent
late or non-payment on your account may result in dismissal from the
practice.
