Insurance
co-payments are due at each appointment.
Full payment of $90 is due
at each appointment if you do not have insurance coverage. While we are not accepting new cash-pay patients, patients who are currently paying cash, as well as existing patients who lose their insurance coverage, will be charged this flat rate plus 60% of any tests or procedures performed and billable in our office.
We
accept cash, MasterCard and Visa, and personal checks.
A
$25 fee will be charged on all returned checks.
You are responsible to call our office to work out a payment plan if you cannot pay your bill. Please ask for Debbie, who will be glad to assist you.
Persistent late payments or non-payment on your account may result in dismissal from our practice.
